I understand and agree that any misrepresentations by me in this application will be sufficient cause for cancellation of this application and/or separation from YES to YOUTH - Montgomery County Youth Services if I have already been employed/volunteering.  I understand that YES to YOUTH does not unlawfully discriminate on employment and no question on this application is used for the purpose of limiting or excusing any application from consideration for employment on a basis prohibited by applicable local, state, or federal law.

I authorize YES to YOUTH - MCYS to investigate all references and to secure additional information about me, if job related, from all references (personal and professional), employers, public agencies, licensing authorities, and educational institutions and to otherwise verify the accuracy of all information provided by me in this application, resume or job interview.  I hereby release YES to YOUTH - MCYS and its representatives from liability for seeking such information and all other persons, corporations, or organizations for furnishing such information.

I understand that this application remains current only for 30 days.  At the conclusion of that time, if I have not heard from YES to YOUTH - MCYS and still wish to be considered for employment, it will be necessary to reapply and fill out a new application.

I understand that nothing in this application, or in any prior or subsequent written or oral statement, creates a contract of employment or any rights in the nature of a contract.  I agree and understand that if I am hired by YES to YOUTH - MCYS, my relationship will be at-will, for an indefinite period of time, and may be terminated at any time, with or without cause or notice, at the option of YES to YOUTH - MCYS or myself.  I understand that I have the right to en my employment at any time and that YES to YOUTH -MCYS retains that same right.  I also understand that no one has the authority to enter into any contract, agreement or modifications of the foregoing unless such contract, agreement or modification is in writing and signed by the Chief Executive Officer of YES to YOUTH - MCYS.

I also understand that if I am hired by YES to YOUTH - MCYS, I will be required to provide proof of identity and legal authority to work in the United States and that immigration laws require me to complete an I-9 Form in this regard.

In addition, I understand that if I am hired by YES to YOUTH - MCYS, I will have to successfully complete a probationary period and pass a criminal background check as set forth int he Texas minimum standards for a general residential organization as a requirement of my continued employment.
